The content on this website is for general informational and educational purposes only. Reviewing this website or contacting our administration desk via phone, email, or web form does not establish a formal doctor-patient relationship. A formal relationship is only created once you have been formally accepted, intake is complete, and you are seen by one of our licensed healthcare practitioners.
3. STRICT UNENCRYPTED COMMUNICATION PROHIBITION
Our website contact forms and general email addresses are intended for basic administrative inquiries only (e.g., clinic hours, general services offered). They are not secure or encrypted. You strictly agree NOT to submit sensitive personal health information, medical histories, symptoms, or medical photos through our website forms or standard email. The clinic accepts no liability for privacy breaches resulting from information you choose to send over unencrypted channels.
4. OHIP AND UNINSURED SERVICES
Our clinic provides a combination of publicly funded and private healthcare services:
- OHIP Services: Valid Ontario Health Insurance Plan (OHIP) coverage must be presented at every visit. If your card is expired, invalid, or you do not bring it, you may be charged a temporary out-of-pocket fee, which will be refunded once valid coverage is proven.
- Uninsured Services: Services not covered by OHIP (including but not limited to treatments, third-party forms, missed appointment fees) are the sole financial responsibility of the patient. Payment for uninsured services is strictly due at the time the service is rendered or booked, as communicated by our administrative staff.
5. MANUAL APPOINTMENT & CANCELLATION POLICY
All appointments are scheduled manually by our administrative team. It is your responsibility to confirm your scheduled date and time with our staff. If you need to cancel or reschedule, you must provide at least 24 hours’ notice directly to our office by phone or email. Failure to provide adequate notice or failure to show up for your appointment will result in an uninsured “No-Show” fee which must be paid before any subsequent appointments can be booked.
6. GOVERNING LAW
These terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein. Any legal disputes arising from the use of this website must be brought exclusively before the courts of Ontario.
